2. Following payment, we will email a bio questionnaire for each doctor - please provide the name(s) and email address for each doctor.
 
3. A photo session will be scheduled (at doctor's office)
 
4. We will notify you when the Premium profile is published 

Payments are processed as a monthly subscription via PayPal Recurring Payments. You will be charged the monthly subscription fee automatically each month.
