Notice For Old Dominion Osteopathic Medicine Patients

It is with a heavy heart that I am writing to say goodbye to all of you. As a result of the economic impacts of COVID 19, my business is not going to survive, despite the various government assistance programs. I have taken a new position in Louisiana and will be moving in the next few months.

Therefore, Old Dominion Osteopathic Medicine will close effective June 5, 2020.  I will continue to see patients in the office until May 21, 2020, but you will need to identify a new provider to provide your continuing medical care. 

I recommend:

  • Dr. Michael Porvaznik in Arlington (703-931-7173)

  • Dr. Gregory Craddock in Falls Church (703-241-1033) 

  • Arlington Osteopathic General Practice (703-522-8404)

  • Osteopathic Family Physicians in Alexandria (703-998-6760)

You have several options in regards to your medical records maintained in our office:

  1. You can pick up your medical records at the practice (Please give us at least 2 business days’ notice so we can    have them ready for you)

  2. You can request that your medical records be mailed to you

  3. You can request that your medical records be forwarded to the office of another healthcare provider.

Please contact our office by June 5, 2020 if you would like to receive your medical records or have them forwarded to another provider – there will be no charge for these requests. Our current hours are Monday-Thursday, 8am-12pm. If you call us after hours please leave a voicemail and we will return your call on the next business day.

After 06/05/2020, Morgan Records Management, 159 Frontage Rd, Manchester, NH 03103, will be the official custodian of these records for the years 2000‐2019. You may request a copy of your records, for a nominal fee paid to Morgan Records Management by the patient, by visiting: MorganRecordsManagement.com, then click on the Medical Records tab.
